You spawn at the bottom of a structure. You'll spend about 3-5 minutes climbing up, moving from platform to platform. The path isn't always straight up, so you have to look around to find the next hold. Eventually, you hit a boss. You don't fight them with weapons, really; you just get past them to reach the Brainhot. If you fall, you respawn a bit lower down, which isn't a huge penalty but can get annoying on higher levels.
